Short-Term Rehabilitation

Diakon offers physical, occupational and speech therapies, as well as a broad range of specialized short-term rehabilitation programs, including orthopedic, neurological and pulmonary rehabilitation. Skilled therapists create individualized therapy plans to help patients achieve their goals, strengthen their bodies and return to the activities they most enjoy.

Short-Term Post-Acute Care

We provide treatment and care for those with complex medical conditions. Our care teams’ multidisciplinary expertise allows us to manage a variety of diseases and health conditions such as diabetes, cancer, postsurgical complications and chronic renal failure, as well as vascular and neurological disorders. The goal of this program is to help patients transition from the hospital back to their regular lives by providing the highest quality nursing and rehabilitative care.

Direct Admissions

We are Medicare-certified and accept admissions from outside our community 24 hours a day, seven days a week.
